Faces Lose Volume, but Lower Eyelids Get Fatter with Age, Study Finds
Have you ever wondered how those pesky under-eye bags got there? This study examined why lower eyelids get bulgy with age by looking at the faces of people in three different age groups (12 – 29 years old, 30 – 54, and 55 – to 80) via magnetic resonance imaging (MRI). In the study, plastic surgeons discovered that while the rest of the face loses volume with age; lower eyelids actually get fatter. This significant increase in orbital fat is the primary cause of under-eye bags. Previous studies have hypothesized that the “bags” were caused by swelling from fluid buildup or sagging/drooping of the eyelid/upper cheek area. This study confirms that excision or removal of the fat, during eyelid surgery, not just repositioning, plays a clear role in the treatment of under eye bags. More than 240,000 eyelid surgeries were performed in 2007, according to ASPS statistics. Eyelid surgery was the fourth most popular Los Angeles cosmetic surgery procedure last year. Plastic Surgery after Gastric Bypass: Who’s Doing It?
This study looked at the socioeconomic factors influencing post-bariatric surgery patients in their decision to have body contouring as their Palm Springs plastic surgery. Although a previous PRS journal study found that the majority of post-bariatric patients want to have body contouring; this study revealed only 11.3 percent of gastric bypass patients actually have body contouring. Additionally, this study found that patients further out from their gastric bypass surgery, with a larger change in BMI were more likely to have body contouring. Of the 105 patients who had body contouring after gastric bypass surgery, 35 patients paid out-of-pocket for their surgery and 68 had insurance that covered all or some of their initial plastic surgery. Nearly 67,000 body contouring procedures after massive weight loss were performed in 2007, up 2 percent since 2006, according to ASPS statistics. Augmentation of the breast consists of insertion of a silicone bag (prosthesis) under the breast (submammary) or under the breast and chest muscle (subpectoral) and then filling the bag with saline (salt water). A Los Angeles breast augmentation expands the breast area to give a fuller breast (increased cup size), give a better contour, and give more cleavage.
The incision (cut) may be placed at various locations such as: under the breast at the breast fold; periareolar which is at the edge of the areola (brown area consisting of nipple and areola); within the areola; in the armpit (axillary); or in the umbilical area.
The thinnest scar is usually in the areola. The umbilical incision approach is used with an endoscope (tube with a light for visualization and placement of the breast implant).
The smooth surface implant requires a large pocket and is associated with a 10% incidence of capsule contracture (tightening of the scar around the implant causing firmness or hardness). The smooth implant has only an occasional occurrence of rippling or wrinkling which gives the overlying skin a wavy appearance.
A textured (roughened surface) implant is associated with a 3% incidence of capsule contracture but has a 7-9% occurrence of rippling.
What are the choices of implant positions? The easiest surgical placement of the implant is under the breast tissue (submammary).
A slightly more difficult surgical placement is under the chest muscle (submuscular or subpectoral) but this position is associated with a lower incidence of capsule contracture and slightly less occurrence of rippling. There is more pain postoperatively than with a submammary placement.
The ultimate size of the breast is mainly a personal decision by the patient. Cup size is an inexact estimate of the final size since cup sizes vary with the bra manufacturer and how tight or loose the patient wears her bra as well as how much breast tissue is already present. Describing the preferred cup size does aid the surgeon, but it also helps to look at pictures or photos to show the California plastic surgeon the desired final look.
The most accurate method of estimating the implant size (in ounces) is to fill a plastic bag with measured amounts of water and place in a bra with the patient’s estimated cup size. One can see with the bra on and covered by a sweater what the final result will approximate.

In some instances, your plastic surgeon may recommend a baseline mammogram before surgery and another mammographic examination some months after surgery. This will help to detect any future changes in your breast tissue. Following California breast augmentation you will still be able to perform breast self-examination.
If you are a smoker, you will be asked to stop smoking well in advance of surgery. Aspirin and certain anti-inflammatory drugs can cause increased bleeding, so you should avoid taking these medications for a period of time before surgery. You may be permitted to go home after a few hours, unless you and your plastic surgeon have determined that you will stay in the hospital or surgical facility overnight.

The septum can be deviated at birth (congenital) or because of injury, such as a broken nose. It also may bend to one side or the other as a result of aging. Very few people have a perfectly straight septum. Septoplasty generally is necessary only when breathing problems or snoring do not get better without surgery. The doctor trims or straightens the bent cartilage and then replaces the mucosa over the cartilage and bone. The procedure is often done along with a rhinoplasty.

Laser-assisted liposuction, introduced in 2007, is one of the newest liposuction techniques. SmartLipo is the first laser to be approved by the FDA to be used for fat dissolution.
According to FDA documents, the SmartLipo laser is intended for the surgical incision, excision, vaporization, ablation, and coagulation of soft tissue. While all soft tissue is included, it is also indicated for laser-assisted lipolysis (liposuction).
This laser technology produces a burst of energy as it enters the fatty tissue that liquefies the fat. Similar to other techniques of liposuction, laser-assisted liposuction requires the use of tumescent solution prior to lipolysis, and standard liposuction to aspirate the liquefied fat.
The laser energy is delivered by a thin laser fiber that is inserted through small incisions in the skin. In addition to breaking up fat cells, the laser energy also heats other tissues. Possible side effects include, but are not limited to, swelling, bruising, burns, numbness and minimal bleeding.
The laser for use in liposuction is deemed a safe and effective device by the FDA. The only study that has scientifically compared this to standard liposuction has found no differences between these techniques in terms of the results. The best procedure for a patient depends on many factors which is why a thorough consultation with a plastic surgery specialist is beneficial. Sometimes a Beverly Hills tummy tuck is a better choice than liposuction.
